What do I need to bring on my first visit?

  • Physical therapy prescription or referral if you need one
  • Health insurance card if you have one
  • ID
  • Be prepared to provide your previous medical or surgical history
  • If they are relevant bring scan/MRI report or X-ray report
  • You may wish to bring shorts and/or t-shirt
  • Your running shoes if your injury involves running

Where are you located?

Our address is 483 Pine Street, Financial District of San Francisco.  The entrance is on Pine Street at the north end of Belden Place.  We are located between Kearny and Montgomery with Kearny being the closer main street.  The entrance is a street level grey door underneath a grey awning between two restaurants (Fusion and Bryndisi).

Transportation

Belden Physical Therapy and Fitness is easily accessible by public transportation (BART, Muni).

Parking: If you must drive, it's Financial District and very hard to find parking.  There are a number of parking garages nearby.

Bikes: There are bike racks right out front, have a very good bike lock.  You are always welcome to bring your bike in, you just have to carry it up the staircase.

What should I wear to my appointment?

  • We have a changing room/bathroom so bring comfortable clothes you can change into
  • If you are being seen for your
    • foot/ankle: clean feet
    • knee: shorts
    • hip: shorts are preferrable
    • back: loose clothing
    • shoulder: loose t-shirt or sleeveless shirt
    • neck: loose shirt and remove jewelry
    • hand/wrist/elbow: shirt that can be rolled up

Do I have to be referred by a doctor?

No, physical therapists can assess and treat without referral by a doctor for 12 visits or 45 days before having to go to a physician for an examination. However most health insurers require a doctors referral before they authorize coverage of fees, some only require a phone call.  If you need help with understanding your health insurance coverage for physical therapy, please let us know.

 

We see and treat patients for a wide range of conditions and diagnoses.  Please visit our Services for an extensive list of conditions and diagnoses we see patients for.

We will check with your health insurance provide to determine if outpatient physical therapy services are covered under your plan. We recommend that you confirm  your insurance coverage.

You may call our office at (415) 872-9376 or email admin@beldenpt.com.  We have a 24-hour late cancellation/rescheduling fee that is waived once.  We understand that there will always be circumstances that may not be under your control, and we do not like charging you this fee so please do not make a habit of cancelling/rescheduling.

We have no shower facility but you can freshen up in our unisex bathroom.

We are currently going through insurance credentialing with a number of carriers so please check in periodically to see if we take your insurance. 

Using insurance is more common for physical therapy, however it is not uncommon to go out-of-pocket.  Please inquire about our cash rates and benefits.

Initial evaluations usually require 45 minutes to an hour.  Follow up sessions may require 30 minutes to an hour.  In general, please allow up to an hour for your visits.  If you have time constraints, please inform us at the beginning of the session or earlier so we can plan accordingly. 

The number of treatments will vary depending on the problem and individual. After a thorough assessment, the physical therapist will discuss their treatment plan for the rehabilitation process.
